Who is transferring?

I have read and agree to the Labyrinth BJJ liability waiver and membership terms. I authorize Labyrinth BJJ to store this card and charge it for my monthly membership, replacing my Team Legacy billing. Nothing is charged today. Team Legacy takes its final payment in September, and my Labyrinth billing starts in October. The rate is the one we agree for the plan we choose together, and Labyrinth will confirm it with me before the first charge. It then repeats monthly on the same date. Membership is month to month and I can cancel at any time by telling the academy.
